- Đề nghị mở tài khoản (Biểu mẫu TC-02/HSTTKH);
- Quyết định thành lập tổ, đội trực thuộc của tổ chức;
- Văn bản uỷ quyền mở và sử dụng tài khoản của Người đại diện theo pháp luật theo quy định cho người đứng đầu tổ, đội trực thuộc, trong đó nêu rõ: Tên tài khoản của tổ, đội; Phương thức sử dụng dấu, chữ ký trên chứng từ giao dịch với ngân hàng.
