Trưởng đoàn đánh giá tổng hợp các ghi nhận trong quá trình đánh giá, lập báo cáo đánh giá nội bộ theo biểu mẫu BM/ISO-03-05, trong đó thống kê toàn bộ các điểm không phù hợp phát hiện được, đồng thời nhận xét tổng thể về mức độ phù hợp của phòng ban được đánh giá so với tiêu chuẩn được áp dụng. Báo cáo đánh giá nội bộ phải được hoàn thành và gửi cho tất cả các phòng ban được đánh giá trong vòng 7 ngày làm việc sau khi hoàn tất đợt đánh giá. Trưởng đoàn đánh giá chịu trách nhiệm gửi báo cáo cho ban tổng giám đốc và trưởng các phòng ban có liên quan.
Trưởng đoàn đánh giá xét xem phòng ban được đánh giá có sự không phù hợp hoặc lỗi nhắc nhở hay không. Nếu không có thì đóng báo cáo đánh giá chất lượng nội bộ rồi gửi cho đại diện lãnh đạo về chất lượng và ban tổng giám đốc. Nếu có thì thông báo cho phòng ban có liên quan, yêu cầu phòng ban đó phải xác định nguyên nhân và thực hiện hành động khắc phục.
