Bộ mẫu biểu báo cáo cấp chủ

Bộ mẫu biểu báo cáo cấp chủ dự án gồm các mẫu biểu báo cáo quý I, 6 tháng, quý III và cả năm theo Mẫu IV-GSĐG 1, báo cáo giám sát đánh giá trước khi khởi công theo Mẫu IV-GSĐG 2, báo cáo giám sát đánh giá khi điều chỉnh dự án theo Mẫu IV-GSĐG 3 và báo cáo kết thúc chương trình, dự án theo Mẫu IV-GSĐG 4 trong Phụ lục IV của Thông tư này. Ban quản lý dự ánphải xây dựng và trình các báo cáo nêu trên lên chủ dự án để chủ dự án gửi các báo cáo này cho cơ quan chủ quản và các cơ quan liên quan

The form-level report server project contains the form reported in the first quarter, six months, quarter and entire year under model IV-GSĐG 1, monitoring report reviews before starting form IV-GSĐG 2, monitoring report reviews when adjusting the project under model IV-GSĐG 3 and end the program report the project, under model IV-GSĐG 4 in annex IV of this circular. Ánphải project management and construction process of the aforesaid report up to Project Server Project Server to send these reports to the governing body and related agencies

Results (English) 2:

Ministry report forms all project-level reporting forms including the first quarter, six months, quarterly and annual reports on Form III IV-M & E 1, monitoring and evaluation report before starting on Form IV-M & E 2, monitoring and evaluation report adjusted fourth-project M & E on form 3 and program completion reports, project M & E on form IV-4 in Appendix IV of this Circular. PMUs anphai formulate and submit the above report to the project owner for the project owner to send these reports to the governing body and the relevant agencies
