- Đối với từng vị trí công việc quy định tại Mẫu này thì nhà thầu phải kê khai các thông tin chi tiết theo Mẫu số 15 và Mẫu số 16 Chương này.
- Nhà thầu phải kê khai những nhân sự chủ chốt có năng lực phù hợp đáp ứng các yêu cầu quy định tại điểm a Mục 2.2 Chương III – Tiêu chuẩn đánh giá HSDT và có thể sẵn sàng huy động cho gói thầu; không được kê khai những nhân sự đã huy động cho gói thầu khác có thời gian làm việc trùng với thời gian thực hiện gói thầu này. Trường hợp kê khai không trung thực thì nhà thầu sẽ bị đánh giá là gian lận.
