Điều 14. Thay đổi, bổ sung thông tin khách hàng cá nhân
1. Thay đổi giấy tờ tùy thân:
a) Thay đổi tên:
- Giấy đề nghị thay đổi, bổ sung thông tin;
- Văn bản xác nhận việc đổi tên của chủ tài khoản do UBND xã, phường nơi đăng ký khai sinh của chủ tài khoản cấp hoặc văn bản xác nhận việc thay đổi CMND do thay đổi tên của cơ quan công an nơi cấp, đổi CMND;
- Giấy tờ tùy thân mới theo quy định tại Điểm b, Khoản 1, Điều 9 Quy định này.
