1. Giấy đăng ký kiểm tra nhà nước về chất lượng hàng hóa nhập khẩu
2. Bản photo copy các giấy tờ sau:
- Contract
- Packing list
3. Một hoặc các bản sao chứng chỉ chất lượng (có chứng thực)
4. Các tài liệu có liên quan:
- Copy Bill of Lading;
- Invoice;
- Tờ khai hàng hóa nhập khẩu;
- C/O-Certificate of Origin (nếu có)
- Ảnh hoặc bản mô tả hàng hóa; mẫu nhãn hàng nhập khẩu đã được gắn dấu hợp quy và nhãn phụ nếu nhãn chính chưa đủ nội dung theo quy định

d. Thời hạn giải quyết: 03 ngày làm việc kể từ ngày tiếp nhận hồ sơ đăng ký kiểm tra chất lượng.
e. Cơ quan thực hiện thủ tục hành chính: Chi cục Tiêu chuẩn Đo lường Chất lượng các tỉnh, thành phố
g. Kết quả thực hiện thủ tục hành chính: Thông báo kết quả kiểm tra nhà nước về chất lượng hàng hóa nhập khẩu.
h. Lệ phí: theo quy định của Bộ Tài chính
